Comparison of Weil-Felix Test and Indirect Immunoperoxidase Test for the Diagnosis of Tsutsugamushi Disease

It was only in 1985 that tsutsugamushi disease was first confirmed among Koreans, but it was soon revealed that the infection being more prevalent than haemorrhagic fever with renal syndrome or leptospirosis. These infections can not be definitely differentiated clinically. Serologic tests for tsutsugamushi disease include Weil-Felix(W-F) test, indirect immunofluorescent antibody(IFA) test and indirect immunoperoxidase(IIP) test. As wer did not have experience with these tests in Korea, the authors compared the tests. W-F test detected only about one fifth of those detected by IIP. IIP was positive in most patients at about one week of illness. IIP titers were similar to those of IFA. In a patient IgG antibodiy was detected even after 5 years. Some of the nontsutsugamushi disease subjects had low titers of IgG or IgM antibody. Compared to IFA procedure, IIP test required another step of staining with peroxidase substrate, and the substrate and conjugate were not very stable. Advantage of IIP were observing the slide with common light microscope instead of fluorescent one and the slide being kept permanently.
