Clinical Observation on Vibrio Vulnificus Infection and Antimicrobial Susceptibility

A clinical observation was made on 13 cases of Vibrio vulnificus infection from Sep. 1980 to Sep. 1987. Minimal inhibitory concentration (MIC) of 11 antibiotics against 7 clinical isolates of Vibrio vulnificus was determined by Auto-Microbic System-GNS (VITEC System).
The results were as follows.
1) The over all sex ratio of Vibrio vulnificus infection was 11:2 (male: female) with male predominance. The mean age of the patients was 47(range 27~68).
2) The outbreak of the disease was during the summer months (June-September). Most cases occurred during August and September.
3) 4 cases (57%) had history of raw seafood ingestion. 11 cases (85%) had the antecedent liver disease and 4 cases (31%) were heavy drinker.
4) All patients had primary septicemia. Skin lesions were observed in 85% (11/13 cases ) and the location was both the upper and lower extremities in the most cases (6/10 cases).
5) The majority of patients (69%) died of septic shock within 2~7 days (mean 4 days) after onset. Mortality rate seemed to to be higher in leukopenic group than leukocytosis group, but there was no statistical significance. (p>0.05 by Fisher's exact method)
6) Against 7 clinical isolates of Vibrio vulnificus, tetracycline, ampicillin, chloramphenicol, gentamicin, tobramycin, amikacin, cefamandole, trimethoprim-sulfamethoxazole showed good antibacterial activity. However cefoxitin and cephalothin showed poor antibacterial effect.
