Clinical Analysis of Streptococcal Bacteremia from 1992 to 1993 in St. Mary's Hospital

Background : Streptococcal infections have been increased in the immunocompormised patients and the spreads of penicillin-resistant streptococci are trublesome all over the world. We analysed streptococcal infections in St Mary's Hospital for last two years to evaluate the changing patterns in terms of epidemiologic and clinical features.
Methods : We performed a retrospective analysis of the 80 patients with streptococcal bacteremia in St. Mary's hospital from 1992 to 1993.
Results : Overall frequency of streptococcal bacteremia showed increasing trend from 35 cases in 1992 to 45 in 1993 and especially, in the hemato-oncology unit, there was a remarkable increase from 17 cases to 30. Respiratory tract was the most common portal of entry, while the source of bacteremia remained undetermined in 43.8% of patients. The most common species was viridans streptococci, followed by enterococci and Streptococcus pneumoniae. There was no vancomycin-resistant enterococci isolated, although it is concerned that vancomycin-resistant strains may emerge in Korean.
Conclusion : As the current prophylactic antibiotic regimens are mainly directed against gram negative bacilli, they may not be enough to prevent streptococci actively. Therefore, we suggest that it is needed to cover gram positive cocci as well as gram negative bacilli in the prophylatic and empirical therapies for infections in immunocompromised patients.
