E. coli Bacteremia in Cancer Patients

Background : Echerichia coli bacteremia was common in patients with malignancy. The many reviews of gram-negative bacillary bacteremia have been published, but few studies have focused on E. coli. The common clinical features, efficacy of antimicrobial therapy, important prognostic factors of E. coli bacteremia in cancer patients should be determined.
Methods : We reviewed the medical records of fifty cases of E. coli bacteremia in cancer patients retrospectively during a 7-year period(from 1988 to 1994) to investigate the underlying diseases, source of infection, clinical manifestation, effect of antimicrobial therapy, mortality and prognostic factors.
Results : Fever occurred in 94% of patients. Only 4% of the patients had disseminated intravascular coagulation. The overall response rate was 64%. Age, hematologic malignancy, response to antimicrobials, shock and DIC were the prognostic factors irrespective of underlying diseases.
Conclusion : The majority of E. coli isolated were susceptible to a wide variety of antimicrobial agents. The appropriately selected antimicrobial agents would provide adequate management against this common pathogen even in cancer patients.
