A Case of Black Hairy Tongue Caused by Candida Species Showing Resistance to Itraconazole and Fluconazole Therapy

Candida and Aspergillus species can frequently isolated from black hairy tongue, and prolonged use of oxidizing agents, excessive use of tobacco, oral or parenteral antibiotic therapy, poor oral hygiene, and radiotherapy are also responsible for the development of black hairy tongue.
We present a case of black hairy tongue caused by C. albicans and C. glabrata showing resistance to long-term itraconazole and fluconazole therapy. The patient was a 80 year old woman who had complained of black-colored, hyperkeratotic, cobblestone-like plaques on the center of the tongue for 10 days. She was managed with itraconazole and fluconazole for 2 years, but the clinical improvement was unremarkable and the mycologic studies continuously revealed positive results.
